HEALTHILY RESEARCH TERMS AND CONDITIONS

The Research (the “Research”) is organised by Healthily Ltd, a company incorporated in England and Wales, with a registered number 8727263 and registered office address at 36 Egerton Road, Bournemouth BH8 9AY (“Healthily”), acting as controller in regard to your data.
Healthily is a digital health company that provides an AI Health Navigation Platform incorporating AI Medical Assessment (medical device class IIa) and Medically Validated Content to help individuals better understand and manage their health (“Healthily Service”). Its AI Medical Assessment is designed to guide users through a structured set of questions about their symptoms and, based on this input, it provides end users with information about the possible types and levels of care and an overview of possible conditions. The AI Medical Assessment is intended to support informed decision-making and improve access to relevant health content, but it does not provide medical diagnosis or replace professional medical advice.
PURPOSE OF THE RESEARCH
The purpose of the Research is to understand how you manage your health, including how you seek information, make decisions, and use health-related tools and services. The findings will be used to inform the design and improvement of Healthily Service (altogether referred to as “Research Purpose”).
Please make sure you do not provide any personal information while completing the Questionnaire. Your identification or specific information about your health is not necessary to perform the Research, so please do not provide any information that may allow us to identify you.
PARTICIPATION IN THE RESEARCH
Participation in Research is available to the Attest Audience. There is no entry criterion for participating in the Research; however, Healthily will aim to achieve overall demographic splits by following general population splits across age, gender, household income, ethnicity, and location. By participating in the Research, you agree to these Terms and Conditions (https://www.healthily.ai/terms-and-conditions) (the “Terms”). QUESTIONNAIRE
The Research comprises 13 questions relating to how you manage your health. As part of the research, you will be presented with an example of an assessment report generated by the Healthily AI Medical Assessment and invited to provide your feedback on its content, clarity, and usefulness.
This Research is conducted using a third-party research platform, Attest, which is used to host the questionnaire and collect responses.
DATA PROTECTION
Healthily is acting as a data controller in regard to your data.
To help protect your privacy, Attest applies pseudonymisation techniques. This means you will be assigned a unique identifier that is used to link your responses with the survey system for operational and analytical purposes. The identifier does not directly reveal your identity.
Healthily does not receive any information that directly identifies you.

GOVERNING LAW
These Terms and Conditions shall be governed by and interpreted in accordance with the laws of England. You and Healthily agree to the exclusive jurisdiction of the courts of England in respect of any disputes which may arise in relation to the Test.
